Our billing department will submit claims to these plans for you.  All applicable co-pays will be collected at the time of service.  Many of these are manged care plans that require a referral from your primary care physician. (please bring this form to our office at the time of service) and/or pre-certification for certain procedures and hospital admissions.  Please contact your plan's membership department if you have any questions.  Our billing department can also assist you with questions regarding participating plan rules.

If you are insured by a plan that we don't participate in, as a courtesy we will submit the claim form from your carrier.  Our billing department will be more than happy to assist you, however, you are responsible for all charges incurred.
